RN Supervisor/Educator

Position Summary:
As an RN Supervisor you will be working and communicating daily with professional, paraprofessional and clients and their families.  You will be accountable to adherence to federal, state, local laws, accreditation and company policies.  As an RN Educator you will be providing educational training to all Personal Care Aide, Home Health Aide and Home Care Support Services, including initial training, transitions, upgrades, in-services and annual competency evaluations.  You will also conduct UAS assessments and Private Duty evaluations as requested by the Director of Clinical Services.   

Position Requirements:
You will need to be a Registered Nurse holding a current license in NY State, and have two years of clinical experience in a home health agency to conduct specific types of supervisory visits.

  • Past experience and educational qualifications are appropriate for the duties assigned and to be responsible for the supervision of patient care. Knowledgeable of nursing and medical practices, procedures, regulations and guidelines that pertain to home health care.
  • Previous experience with NY State regulations and compliance requirements as related to NY State home care preferred.
  • Prefer experience with Medicaid and Insurance requirements. 
  • Must possess excellent organizational, interpersonal, telephone, documentation and communication skills.
  • Must have communications skills such that information concerning a patient's condition is communicated clearly to professional and paraprofessional staff as well as to the client and their caregivers.
  • Must possess strong clinical assessment, evaluation and patient education skills.
  • Must have ability to operate a computer and/or a tablet PC. Willing/able to be trained on software package used by Agency.
  • Must maintain compliance with in-service, continuing education and medical requirements as set by NY State Regulations, Agency policy and accreditation organization(s).
  • Must possess the disposition and personality to work harmoniously with a wide variety of people and all levels of staff, patients and families. Must have the ability to deal with abrupt schedule changes and to maintain objectivity in coping with stress of working with acute, chronic and terminally ill people.

Working Environment / Physical and Personal Requirements:

  • Able to read, write and communicate in English.
  • Pass a physical examination after being offered employment and a health assessment annually thereafter.
  • Must be able to complete all physical demands of the job, which may include, but not be limited to: the ability to move up to 50 pounds, perform one person transfer under routine and emergency situations, reposition a bed bound patient alone and able to sit, stand, bend, stoop, kneel, pull, climb, reach, lift and perform repetitive movements of the upper extremities. Must be able to read normal typewritten print and have corrected vision and hearing within normal range. Has manual dexterity with normal range of motion of all extremities.
  • Field experience may expose employee to temperature extremes, physical injury by patient, family, animal or other in the community, unsafe environment or exposure to infectious diseases and offensive odors. 
  • Is required to occasionally work weekends, holidays, and variable hours. Required to share on-call responsibilities on a rotating basis.
  • Must be able to work independently setting own priorities based upon approved goals with minimum supervision. Must be able to multi-task.
  • Possess a valid driver's license, automobile liability insurance and have a car available for work use as driving to /from client homes to conduct visits is an essential function of the position.
  • Must have access to a working phone for communication to/from Health Force office staff. 

Supervisory Responsibilities

  • Supervises RNs, LPNs in the adherence to, understanding of and provision of care based on the patient's Plan of Care.
  • Provide direct supervision and direction and/or indirect supervision and direction to paraprofessional staff in the adherence to, understanding of and provision of care based on the patient's Care Plan.
  • Provide on-the-job training as needed to paraprofessional and professional staff members to ensure competency of ordered tasks/care to be provided.
